Common Benefits Terms

Plain-language explanations for the terms you'll encounter most in your benefits documents.

A deductible is the amount you pay out-of-pocket for covered health services before your insurance begins to pay. For example, with a $1,500 deductible, you cover the first $1,500 of eligible costs each year. After that, your plan starts sharing the expense. Preventive care is typically covered before you meet your deductible.
A copay is a fixed dollar amount you pay at the time of a covered visit — for example, $20 for a primary care appointment or $50 for a specialist. Copays are usually separate from your deductible and stay the same each visit regardless of the total cost of the service.
Coinsurance is the percentage of costs you share with your insurer after you've met your deductible. With 80/20 coinsurance, your plan pays 80% of covered costs and you pay the remaining 20%. You continue sharing costs at this rate until you reach your annual out-of-pocket maximum.
The out-of-pocket maximum is the most you'll ever pay for covered services in a plan year — including your deductible, copays, and coinsurance combined. Once you hit that ceiling, your plan covers 100% of covered costs for the rest of the year. It's your financial safety net for high-cost medical situations.
A premium is the monthly amount you pay to keep your insurance active, whether or not you use any services that month. If your employer contributes to the cost, you'll typically see your share deducted from your paycheck. Keeping your premium current ensures your coverage stays in force.
In-network providers have negotiated contracted rates with your carrier, so your plan covers a larger share of the cost and your out-of-pocket is lower. Out-of-network providers haven't agreed to those rates, which means higher costs or potentially no coverage at all. Always confirm a provider is in-network before your appointment to avoid surprise bills.
An EOB is a summary your insurer sends after you receive covered care. It details what was billed, what your plan paid, and what you may owe — but it is not a bill. Review your EOB carefully to confirm the services match what you received and to understand exactly how your benefits were applied.
